چکیده
was referred to the neurology department for comatose state with brainstem dysfunction. Non-contrast brain CT showed diffuse cerebral edema with subarachnoid hemorrhage (SAH)-like appearance (Fig. 1A). A lumbar puncture was not carried out because of the brain edema which was revealed in CT examination. T2*-weighted gradient-echo MRi revealed enlarged subarachnoid vessels that were not consistent with true SAH (Fig. 1B). Final diagnosis was septic encephalopathy with pseudo-SAH. Pseudo-SAH has been reported in association with cerebral edema, bacterial meningitis, subdural hematoma, venous sinus thrombosis and spontaneous intracranial hypotension (1, 2). The presence of pseudo-SAH in diffuse cerebral edema is believed to be due to venous congestion and distention (1, 2). Venous congestion can increase the amount of deoxy hemoglobin by decreasing blood flow. Therefore, gradient echo MRi may be helpful in distinguishing pseudo-SAH from true SAH.

Detection of subarachnoid haemorrhage with magnetic resonance imaging.
OBJECTIVES To measure the sensitivity and specificity of five MRI sequences to subarachnoid haemorrhage. METHODS Forty one patients presenting with histories suspicious of subarachnoid haemorrhage (SAH) were investigated with MRI using T1 weighted, T2 weighted, single shot fast spin echo (express), fluid attenuation inversion recovery (FLAIR), and gradient echo T2* sequences, and also by CT. ...
متن کاملMR imaging of hyperacute subarachnoid and intraventricular hemorrhage at 3T: a preliminary report of gradient echo T2*-weighted sequences.
We describe MR imaging findings applying gradient echo (GRE) T2*-weighted and fluid-attenuated inversion recovery (FLAIR) MR images at 3T to three patients with hyperacute subarachnoid and intraventricular hemorrhage from ruptured aneurysms. Hyperacute subarachnoid and intraventricular hemorrhages (SAH and IVH) were more clearly visualized as an area of decreased signal intensity on GRE T2*-wei...
متن کاملHyperacute subarachnoid hemorrhage on T2-weighted MR images.
We describe a case of hyperacute subarachnoid and intraventricular hemorrhage from a ruptured aneurysm, which occurred while the patient was undergoing MR imaging. Compared with CSF, the blood in the subarachnoid space had slightly lower signal intensity on T2*-weighted gradient-echo images and increased signal intensity on T2-weighted spin-echo images. This finding differs from the generally a...
متن کاملSuperficial Siderosis of the Central Nervous System Induced by a Single-Episode of Traumatic Subarachnoid Hemorrhage: A Study Using MRI-Enhanced Gradient Echo T2 Star-Weighted Angiography
The purpose of this study was to examine whether a single episode of traumatic subarachnoid hemorrhage (tSAH) could cause superficial siderosis of the central nervous system (SS-CNS).This study was approved by the local ethics committee. Thirty-two patients with a history of a single episode of tSAH were enrolled in the study. An episode of tSAH was confirmed in patients based on a CT scan or a...
متن کاملDetection of subarachnoid hemorrhage at acute and subacute/chronic stages: comparison of four magnetic resonance imaging pulse sequences and computed tomography.
BACKGROUND Acute subarachnoid hemorrhage (SAH) has traditionally been diagnosed by computed tomography (CT); however, fluid-attenuated inversion recovery (FLAIR) is a magnetic resonance imaging (MRI) modality currently used to detect acute SAH. CT is insensitive in the detection of subacute or chronic SAH. The purpose of this study was to compare 4 MRI pulse sequences and CT in the detection of...
